Pain In Th e… SI
Occasionally I write about a low back problem
that causes people to be “crooked”. You
may have experienced this or know someone
who has. It causes people to stand with a
noticeable tilt and it looks like their pelvis is
pushed 4-5 inches sideways.
On X ray a vertebra may actually be tilted
sideways and stuck in that position. We have
many people come here for care of this very
painful condition.
Th ere is another common low back problem
that I want to mention. It involves the
sacroiliac joint (SI for short). Th e SI joints
are at waist level in the low back and about 2
inches from center on the left and right.
In our practice, SI problems may be the most
common cause of low back pain.
Th ey can be extremely painful to the touch
and rising from a sitting position can be
a painful chore. It is common that when
people try to stand they get stuck and have
to “ratchet” themselves up a little at a time.
Th ey oft en comment that is makes them feel
“like I’m 100!” Th e pain oft en radiates down
the leg because the sciatic nerve passes right
under the SI joint. Infl ammation spreads to
the nerves and can mimic a disc problem.
Oft en the SI pain radiates over the top of the
hip bones or straight through to the front of
the pelvis. Th is kind of pain can radiate into
the groin or thigh and can even cause testicle
pain in men.
If you have a “grabbing” or “jabbing” pain in
your right or left low back, with pain when
you move and even pain that radiates to the
leg, pelvis or groin- you may have a sacroiliac
problem and…
• Th is may feel like a muscle spasm,
but it is joint pain.
• Muscle relaxers are oft en prescribed
but seldom work.
• Heat feels good but makes it worse.
• Ice may not feel good when applied but
it reduces the infl ammation.
• Bending and twisting or reaching can
exacerbate it.
• Chiropractic care addresses the
underlying weakness that can make this
a recurrent and chronic problem.
